How CalAIM Is Transforming Lives Through Housing and Whole-Person Care
California is reimagining what care looks like and changing lives in the process.
Through CalAIM (California Advancing and Innovating Medi-Cal), the state is shifting toward a more connected, compassionate system of care. Led by the California Department of Health Care Services, CalAIM focuses on the whole person bringing together physical health, behavioral health, and social services to improve outcomes and advance health equity.
At LifeSTEPS, this vision aligns deeply with our mission: empowering individuals and families to achieve stability, independence, and well-being through supportive services and housing.
Our CalAIM Journey
LifeSTEPS began delivering CalAIM services in late 2022 through a partnership with Anthem Blue Cross. Building on that foundation, we expanded our reach in 2023 through a PATH CITED grant allowing us to serve even more Californians facing complex challenges.
Today, we proudly partner with L.A. Care Health Plan, Kaiser Permanente, Molina Healthcare, and Health Net.
Our growing CalAIM team now includes 14 Lead Care Managers, administrative support, and program leadership—serving members across Alameda, Los Angeles, Sacramento, Riverside, and San Bernardino counties. With continued expansion planned for 2026, our impact is only accelerating.
What LifeSTEPS Provides Through CalAIM
Enhanced Care Management (ECM)
Enhanced Care Management provides intensive, personalized care coordination for individuals with complex medical, behavioral, or social needs. Each participant is paired with a dedicated Lead Care Manager who helps them navigate healthcare systems, access community resources, and build a path toward stability.
Housing Community Supports
Housing is health and CalAIM recognizes that.
Through Housing Community Supports, LifeSTEPS helps individuals secure and maintain stable housing by offering:
- Housing
- Navigation
- Application and documentation support
- Financial
assistance (such as security deposits) - Ongoing tenancy support
- Landlord communication and eviction prevention
